COMMUNITY HEALTH WORKER
Red lake homeless shelter-community health service program

Position Summary:
The Red Lake Homeless Shelter’s “Homeless to Homes Programs” is a Housing Support program developed to address multiple barriers facing individuals who are currently housed/unhoused. The Community Health Worker (CHW) will work with the vulnerable population in providing education and support for health-related topics that may have historically created barriers for individuals who are homeless or at risk of homelessness. The position will require working in an office setting as well as making home visits to individuals participating in the program. The position may require working outside traditional office hours to possibly include weekends and holidays. The person in this position must be able to work with individuals with a wide range of backgrounds and demographics. Majority of time will be spent meeting with clients and effectively managing time. Reports to Executive Director/ Red Lake Community Health Services Program Director, Full time position, w/benefits. Salary; DOQ.

ESSENTIAL RESPONSIBILITIES/DUTIES:
This position will be responsible for working under the supervision of a Medical Assistance enrolled physician, registered nurse, advanced practice registered nurse or mental health professional and will be responsible for the coordination of the initial visit to establish Community Health Worker services. The position will also be responsible for:
Provide community-based services to eligible adult clients (age 18+) with health conditions that impacts their ability to live successfully in the community.
Assist individuals with the development of health goals and plans.
Coordinate and refer individuals for appropriate medical needs and attend appointments upon request.
Develop educational health related materials and curriculum to
Document services and visits in appropriate electronic health records system.
Communicate important information pertinent to the client’s Housing Case Manager and other professionals working with the client.
Schedule and attend appointments with participants in program.
Motive individuals to be active, engaged and participants in their health.
Coach patients in effective management of their chronic health conditions and self-care.
EDUCATION/WORK EXPERIENCE/QUALIFICATIONS:
At least 18 years old.
High school graduate or equivalent.
Received a certificate from the Minnesota State Colleges and Universities System approved Community Health Worker curriculum.
Willing to obtain first aid and CPR within 6 months of hire.
Valid Minnesota driver’s license and reliable transportation with valid insurance.
Knowledge of established concepts, principles and practices related to housing and homelessness, with full consideration of mental, emotional and physical factors.
Knowledge of American Indian culture and traditions.
Complete mandated reporter training, which includes training on vulnerable adult law.
Ability to use critical thinking skills, function independently and make complex decisions based on standards of care, policies and procedures, critical pathways.
Knowledge of regulatory agencies documentation requirements and ability to accurately and completely document clinical data.
Excellent interpersonal skills in handling interactions with staff, other agencies, groups, and patients and families; capable of working with and actively participating with a diverse multi-disciplinary team.
Associate’s Degree in one the of the Behavioral Sciences or Applied Science Degree - Human Services and two years of experience working in a shelter or community setting. – OR-
